PIAA physical examination form.

PIAA issued changes in the Sport Physical requirements:
One sport physical using the new C.I.P.P.E. (Comprehensive Initial Pre-Participation Physical Evaluation) form can be used for a period of
one year from the date of the exam. A student is required to be
re-examined only if a serious illness or injury has occurred.

Upon submitting your sport physical to the Athletic Director,
you will be able to participate in any sport that falls in the one year
time period from the date of your exam.
