The cost for a copy of a Birth Certificate is $10.00.

To request a copy of a Birth Certificate you can call (412) 565-5113 to hear a recorded message, print out the form
here and mail to the address listed below, or fill out the form online at the Department of Health.

Office Hours:  8:00 a.m. - 4:30 p.m. Monday through Friday

An applicant must be eighteen (18) years of age or older to apply
for a certified copy of his/her birth record or the record
of an immediate family member. The Division of Vital Records
issues only certified copies of birth with the raised seal
that are acceptable for numerous uses, i.e. passport, Social Security,
employment, and personal identification.  


Enrolled students must be five years of age on or before August 31.  Only a parent, foster parent or court appointed guardian may register a child.  If you are a court appointed guardian, you must present documentation.

Required registration information:  birth certificate, immunization records, and child's social security number.  Parents/guardians must provide three proofs of residency (mortgage agreement, rental agreement, tax statement, a current utility bill, and/or driver's license).

Parents/guardians are requested to bring the child to registration for screening for kindergarten and summer school opportunities.

Admission of Beginners

The Board shall establish age requirements for the admission of beginners which are consistent with statute and sound educational practice and which ensure the equitable treatment of all eligible children.

Beginners

Beginners are children entering the lowest grade of the primary school above the kindergarten level.  They shall be admitted to school only during the first two (2) weeks of the annual school term except that a child who is eight (8) years of age may begin school at any time during the school year.

A beginner is eligible for admission to the lowest grade of the primary school above the kindergarten level if she/he has attained the age of six (6) years before the first day of September.

The Board may, on the written request of the parent or guardian, admit as a beginner a child whose chronological age is not less than five (5) years but whose mental age is seven (7) years, when so recommended by a school psychologist and approved by the Superintendent.

Kindergarten

A child is eligible for admission to kindergarten if she/he has attained the age of five (5) years before the admission date of entry to kindergarten.

The Superintendent shall require that each child who registers for entrance to school exhibit one of the following instruments to verify the age of a child:

1.  A legal birth certificate.

2.  A baptismal certificate indicating date of birth, together with a notarized statement from the parent/guardian attesting to the accuracy of the certificate.

3.  A certified copy of entrance into the United States showing the date of birth of the child, along with a notarized statement from the parent/guardian.

4.  Such other evidence as the family Bible record, insurance policy or previous school record showing date of birth of the child, along with the sworn notarized statement from parent/guardian.

5.  If none of the above are available, an affidavit of age sworn by the parent/guardian and accompanied by a certificate of age from a public health officer or public school physician, or in their absence, a licensed practicing physician in the county.
